OUR SERVICES

We provide Community Support to adults, children and adolescents with mental health/substance abuse (MH/SA). This support service consists of MH/SA rehabilitation services and supports necessary to assist the person in achieving and maintaining rehabilitative, sobriety, and recovery goals.

These services are designed to meet the MH/SA treatment, financial, social, behavioral/emotional and other treatment needs of the recipient. We, at AMCH, strive to assist the recipient in acquiring MH/SA recovery skills, social skills and adaptive skills, necessary to successfully address his/her educational, vocational, emotional, behavioral and housing needs.

Our skilled professionals provide coordination of movement across levels of care, directly to the person and their family. We also coordinate discharge planning and community re-entry following hospitalization, residential services, and other levels of care.

We provide "first responder" crisis response on a 24/7/365 basis to consumers experiencing a crisis. Community Support includes case management to arrange, link or integrate multiple services.

Community Support workers provide advocacy and inform the recipient about benefits, community resources and services. We provide assessments for new or existing services in order to meet the recipient's needs.
